Please note the following. We cannot accept emails for consultations or making appointments or give results from tests. The confidentiality of your medical data is important to us that is why certain information cannot be sent back to you because we cannot guarantee who has made the enquiry. You may cancel appointments or any general enquiry can be accepted.

Please note that any electronic communication can never be 100% secure, however, the practice takes every step to ensure that this is the case.

Please provide all the data that identifies you as the patient in the form below. You will be unable to submit the form unless filled in. This is so that we know who you are.
